Accessing Aged Care Services
My Aged Care is the Australian Government's starting point on your aged care journey. Here’s the process for accessing government funded services.
1. Contact My Aged Care
To apply for an assessment for government-subsidised services call My Aged Care on 1800 200 422 or apply online at myagedcare.gov.au This process takes 15-20 minutes to complete, and you will need your Medicare card for ID purposes. If you are applying for someone else, they will need to be present to give their consent.
2.Face to Face Assessment
If you are eligible for support, My Aged Care or an assessment organisation will contact you within 3 weeks to arrange an assessment at your home.
Commonwealth Home Support Program (CHSP)
If My Aged Care refers you for CHSP services a Regional Assessment Service (RAS) assessor will work with you to develop a personalised home support plan. This could include help with housework, personal care, transport, social activities and much more depending on your needs and desires.
Home Care Package (HCP)
If My Aged Care determines that you have more complex care needs an Aged Care Assessment Team (ACAT) assessor will establish the right package level for you. You will receive an approval letter confirming your level of support and you will be placed on a national wait list.
Once your HCP is assigned you will be advised by phone and/or mail.
Your HCP funds can be used for care and services including in-home supports, nursing, mobility aids, home modifications and many more.
3. Find your provider
Your assessor and the My Aged Care team can provide you with a list of available service providers, or you can use the Find a Provider tool on the My Aged Care website. Every provider is different so consider what is important to you, then choose the one that best meets your needs. If you’d like Rise to provide your services you can let your assessor know.
4. The Rise process
If you choose Rise as your provider, once we receive your referral for services, one of our Aged Care Client Coordinators will contact you to book a visit at your house. They will develop a personal, detailed support plan of services with you which addresses your needs and builds on your strengths to help you continue to live as independently as possible in your own home.
5. As things change
It is normal for your care needs to change over time. If this happens you can speak to your Rise coordinator or contact My Aged Care directly to arrange a reassessment so you can continue to live a great life as part of the Rise community.
